Learning Objectives

This module will explain key issues in anticoagulant prescribing including:

  • When a direct-acting oral anticoagulant (DOAC) is the preferred choice of anticoagulant for stroke prevention
  • When warfarin or other vitamin K antagonist is still recommended
  • How to switch patients from a vitamin K antagonist to a DOAC
  • What doses of DOACs to use and the appropriate monitoring
  • How to manage comorbidities and drug interactions when prescribing DOACs
